We are proud to congratulate Stacey Daub, CEO of Toronto Central Community Care Access Centre (Toronto Central CCAC), who recently was awarded The Change Foundation's "20 Faces of Change Award." Stacey has been tireless in leading healthcare innovation at Toronto Central CCAC, and the award represents the persistent effort and great strides made by Stacey and her team to achieve their vision of "outstanding care."

Each year, Toronto Central CCAC connects millions of individuals in need of in-home, clinic and community-based health care services to qualified healthcare practitioners and caregivers throughout Toronto.

In its 20th year running, The Change Foundation introduced the award this year to recognize those working in Ontario healthcare who have inspired positive change. Award winners were chosen by an independent selection committee of healthcare-related professionals.
